Full guidance on ID Check requirements for both the client and the candidate with the documents attached.

Instructions for the candidate

  • Use blue or black ink and write in CAPITAL LETTERS.
  • Fill in Part B of the Disclosure Application carefully, cross checking details on your National Insurance Card, Passport, Driving Licence and/or Birth Certificate as needed.
  • Fill in Part C.
  • Read the Confirming Your Identity document and choose which original identity documents you will present to the organisation that has requested this check.
  • Take the following documents to the organisation that have requested this check.

 

  1. Disclosure Application
  2. Confirming Your Identity document
  3. Chosen original documents

 

Instructions for the client

  • Check that Part B and Part C of the Disclosure Application have been fully and accurately filled in.
  • After you have met with the candidate and have seen their original identity documents, fill in the Identity Check Checklist. You should either write on the checklist what documents were seen or enclose the Confirming Your Identity document when posting the form.
